1. The time sheet is a record of time worked and must be filled out daily.
c. Medical, Dental and Vision Benefits
i. Definitions:
1. Premium Cost
a. Specific types of coverage and benefit payment schedules are described in the City’s health care plan package that is provided to all eligible employees. The City currently pays 90% of monthly premium for enrolled employees. Premium rates are established by the insurance carrier and are subject to change, usually based on increased costs to provide medical services and the amount of services our employees require. If this benefit changes, employees will be notified of the amount the City will contribute towards monthly premiums prior to any changes taking effect.
d. Vacation Benefit
i. All regular employees are eligible for vacation based upon the schedule below. This time shall accrue monthly, but may not be used until the employee reaches their one year anniversary. At the end of the first year, credit is given from the first day of employment. No vacation time may be taken or paid during the introductory period, unless specific arrangements have been made at the time of hire.
ii. Effective for all future hires as of _________, Time is not to be banked and never used; therefore, vacation time may accrue to the level of 12 months total, use it or lose it. Any unscheduled vacation hours accrued above this amount shall be forfeited. Vacation benefits will stop accruing when the maximum allowed has been reached. The benefit will begin accruing again when the total vacation hours are reduced to less than the allowed maximum.
e. Sick Leave Benefit
i. A regular employee will be entitled to sick leave with pay after the first month of employment. Sick leave shall accrue at one day of normal hours per month or 12 days per year. 5 days per year may roll forward for 3 years. Sick leave is accumulated on the last workday of the month. You must be in active pay status on the last working day of the month to accumulate sick leave for that month.
ii. A doctor’s note may be required if an employee misses over three consecutive days of work. A medical release statement may be requested for review before you return to work in certain situations. Unused sick leave is not paid at termination.
f. Paid Holiday Benefit
i. The City of Lyons observes the following holidays each year, during which the office and the library are officially closed:
1. New Year’s Day – Labor Day
2. Memorial Day – Thanksgiving Day
3. Fourth of July – Christmas Day
g. Civic Duty Leave
i. Pay While on Leave
1. You will be compensated based upon your regular rate of pay, but cannot accrue vacation during jury duty
ii. Reinstatement
1. Upon return from jury or witness duty leave you will be reinstated to the same position you held at the time your leave began, subject to our general reinstatement policy. You are expected to report to work during regular work hours when not in court. If requested, you must supply proof of appearance in court for jury service with a note from the court.
